2. United States Department of Transportation (DOT) – Importations of Motor Vehicles and Motor Vehicle Equipment form
3. United States Environmental Protection Agency (EPA) – Importation of Motor Vehicles and Motor Vehicle Subject to Federal Air Pollution Regulations
4. Bill of sale – optional (should have but not required)
5. US Customs and Border Protection -7501 Form
6. Registration
